The siddur was printed in Slavita in 1822 by Rabbi Shmuel Avraham Shapira, son of Rabbi Moshe of Slavita and grandson of Rabbi Pinchas of Koritz.
The first 95 pages of Section A are missing as well as the last five pages (326-330) of Section B.
